STUDENT REGISTRATIONS
FOR STUDENTS RECENTLY MOVING INTO THE CLEARFIELD AREA SCHOOL DISTRICT
STUDENTS WITH ANY CHANGE OF ADDRESS WITHIN THE DISTRICT
TO REGISTER KINDERGARTEN STUDENTS FOR 2012-2013
Parent(s) are invited to stop in their school office over the summer months to register students, if they will be starting out the 2012-13 school year on the first scheduled school day of
AUGUST 29, 2012 .
ALL SCHOOL OFFICES ARE OPEN DURING THE SUMMER. You are welcomed to register students into the District during the summer and avoid any rush of last-minute registering when school begins. We realize that as the beginning of a new school year quickly approaches, families have increased demands on their time.
(We would ask, however, that due to scheduled vacations, etc. for office personnel that you call ahead first to the respective office to ensure that staff is available that day to process your paperwork and arrange for you to tour the building, if desired.)
IF YOU ARE ENROLLING A CHILD FOR KINDERGARTEN :
All children enrolling in Kindergarten for the first time MUST be five years old on or before August 31st of the school year.
You will need the following information to register a Kindergarten student:
Actual Birth Certificate
(or other proof of age)
Immunization Record
Parents must also provide proof of immunization against the following diseases: diphtheria, tetanus, polio, measles, mumps, rubella, hepatitis B, and varicella (chickenpox). Proof of one of each of the immunizations listed is required before entering school.

The following information must be presented at registration:

Student's Social Security Card
