Please submit the required documentation listed below within seven days of registering, so that everything may be finalized and registration is not delayed.  Please send via email to:  czortman@clearfield.org or fax to 814-765-1446.

(If you are having difficulty obtaining the required forms, please contact the elementary school registrar immediately at the number below.  You should still register AS SOON AS POSSIBLE, but we MUST have the required documents prior to the first day of school)

REQUIRED DOCUMENTS:

• Birth Certificate
• Immunizations (Shot record)
• Completed health history (available via online registration @ the link listed above)
• Proof of Residence

(If you do not have a copy of your child’s up-to-date immunizations,

PLEASE have your child’s physician’s office fax it to us at 814-762-8037.)

(Anything with Parent/s name and address on it:  Driver’s license, utility bill, etc.)

Your child MUST be age 5 on or before 08/31/2023, to begin school in the Fall (No Exceptions)

If you do NOT have the means to register on-line from home on your computer or smartphone, please contact the number below as soon as possible to schedule an appointment to do so over the phone with our elementary registrar.
